Retired or separated NCIS Special Agents who meet the good standing criteria established in the Law Enforcement Officers Safety Act (LEOSA) are eligible to receive the DoD Law Enforcement Officer identification card. See the LEOSA Fact Sheet below for additional details.

Requesting personnel must submit a notarized release and waiver of liability agreement, affirmation and acknowledgement for issuance of the DoD law enforcement officer (retired/separated) identification card, and a request for NCIS credentials to Code 11A. These forms are listed below:

Requesting personnel who do not have an electronic photo on file at NCIS Headquarters must submit one with their request.  Photos must be in the ".jpg" format.

In addition, the requesting retired/separated law enforcement officer must also request an Identity History Summary Check through the FBI. The FBI Criminal Justice Information Services offers a few options to submit the request for an $18 fee, yet fingerprints must be obtained locally by the individual at their own expense, prior to submitting.
